This is not a reasonable question to ask; it is like asking whether a
Dodge 4L 6 Cylinder is "DOT compliant".  If installed in an
appropriate support framework with a sturdy body and compliant
interior safety devices such as seatbelts and air bags, it may be DOT
compliant.  By itself, it's a motor, and if you leave it in the garage
on a stand it's certainly DOT compliant.

Pidgin could absolutely be used as part of a HIPAA compliant system.
Out of the box, communicating on a public IM network, it is almost
certainly not.  You would, of course, need to contact a lawyer or the
Department of Health and Human Services to get a legally defensible
opinion on this matter.
